Lincoln County Community Health Assessment Survey

We need your input! Every three years, the Lincoln County Health Department conducts a Community Health Assessment to better understand the health needs, strengths, and priorities of our communities.
The Lincoln County Community Health Assessment Survey is now open!
The assessment provides valuable input from residents, organizations, healthcare providers, businesses, schools, and community partners. The information collected helps identify our county’s most pressing health concerns and serves as the foundation for a Community Health Improvement Plan, which will be developed by the end of this year. Together, these efforts guide local public health priorities, strengthen partnerships, and help inform decisions about programs, services, and future funding opportunities.
The survey will remain open through September 30, 2026. As a thank you for participating, individuals who complete the survey and choose to enter the drawing will be eligible to win one of five $25 gift cards. Winners will be randomly selected and notified after the survey closes, with notification anticipated in early October.
For residents in North Lincoln County, paper copies of the survey will be available at Logan Health Primary Care-Eureka. Individuals interested in completing a paper survey or learning more may contact the clinic at 406-297-3145.
